  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation

  • DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions

  • Truly Board Eligible or Board Certified in Family Medicine, Colorado medical license (or eligibility to obtain)

  • Competitive Base Salary Range: $257,000-$286,000 annually
