- Board Certified in Internal Medicine or Family Medicine (Geriatrics preferred) - Required
- 1+ year experience with frail or elderly populations - Required
- Prior leadership or supervisory experience - Required
- Valid drivers license, insurance, and reliable transportation - Required
- Current BLS, CDS Certification, and DEA Registration - Required
