Clinical Magnet (part of ICON Medical Network), is partnering with a highly respected inpatient rehabilitation hospital system seeking a full-time Director of Case Management to support an exceptional case management team near Parkersburg, West Virginia.

Director of Case Management Responsibilities

• Oversees the case management staff and overall care coordination operations within the rehab hospital


• Manage patient care and discharge plans to improve health outcomes, optimize facility census, and meet key performance goals.


• Develops, monitors, and communicates department schedules, and makes daily adjustments as necessary to optimize patient throughput.


• As a patient and family advocate, you will champion the delivery of personalized care that honors the unique needs and well-being of every patient and their loved ones.


• Build relationships with insurance companies, self-insured employers, case management firms, and other healthcare networks.


• Other duties as assigned

Qualifications

• Clinical RN licensure is not required, although preferred. Social work degrees, licensed clinical social workers and master's degree in healthcare related field considered with relatable experience.


• Registered Nurses must possess bachelor's degree in nursing (BSN) with RN licensure. For other eligible health care professionals, candidate must possess a minimum of a bachelor's degree; a graduate degree is preferred.


• Current, active, unencumbered licensure in state of practice


• Minimum of 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning experience


• Leadership experience


• Certified Case Manager (CCM) or Accredited Case Manager (ACM) designation preferred but must be obtained within 1 (one) year of hire (assistance provided by employer).

Compensation & Benefits

• Compensation based on experience, ranging from $90K - $115K plus quarterly bonus potential
