Founded in 2007, this established and rapidly expanding healthcare organization specializes in caring for geriatric patients residing in assisted living facilities throughout New York.
- Serving patients across 50+ assisted living facilities

Benefits and Features:
$70/hour approximately $145,000 annually
20 Days PTO
$10,000/year Health Insurance Reimbursement
The Role:
Providers conduct video visits from a clean, well-supported clinic environment in Brooklyn. On-site Medical Assistants and med techs at the assisted living facilities assist with vitals and exam preparation, allowing you to focus on clinical decision-making without the noise and unpredictability of traditional facility rounding.
Full-time, Monday Friday, No nights, no weekends- Work out of the Brooklyn clinic conducting video visits with patients in assisted living facilities
- Medical Assistants on-site perform vitals and assist with coordination
- Goal is proactive management of geriatric patients to prevent hospitalizations
- This is an ideal role for a provider who enjoys geriatric medicine and wants high-impact patient care in a structured, efficient setting
Background Desired:
- Licensed Nurse Practitioner or Physician Assistant in New York
- Active DEA License
Must already be credentialed with Medicare and NY State Medicaid
