- Competitive salary: $140,000 $160,000

- Provide comprehensive care in post-acute & long-term care settings
- Perform patient evaluations, diagnoses, and treatment planning
- Order and interpret labs, imaging, and diagnostic tests
- Prescribe medications and therapies
- Document patient care accurately and efficiently
- Educate patients, families, and facility staff
- Work closely with physicians, nurses, and facility leadership
- Participate in interdisciplinary team meetings
- Support quality improvement initiatives and patient outcomes
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) U.S.
- Active or eligible license in:
- Washington
- DEA license (or eligibility)
- Eligible for Medicare/Medicaid participation
- Experience in post-acute, geriatrics, or long-term care preferred
As a Nurse Practitioner in this role, you ll provide compassionate, patient-centered care to individuals in skilled nursing facilities helping improve quality of life and clinical outcomes where it matters most.
