What You ll Do
- Provide comprehensive outpatient care to adult and senior patients
- Diagnose and manage complex medical conditions
- Maintain accurate documentation and coordinate necessary referrals
- Collaborate with an interdisciplinary care team
- Participate in quality improvement and clinical initiatives
Required Qualifications
- MD or DO; active, unrestricted medical license
- Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine
- Minimum 2 years of experience in value based care or high acuity geriatric populations
- Commitment to patient centered, outcomes driven care
- Participation in TB screening
Preferred Qualifications
- Training in Family Medicine, Internal Medicine, Med Peds, or Geriatrics
- Experience in senior care or value based care settings
- Proficiency with EHR systems
- Strong communication and team collaboration skills
- Experience supervising NPs/PAs
