Full-time, mission-driven primary care role providing comprehensive, team-based care to older adults in a PACE setting focused on continuity, prevention, and quality outcomes. Job Overview We are seeking a compassionate Advanced Practice Provider (NP or PA) to deliver high-quality, patient-centered care within a PACE (Program of All-Inclusive Care for the Elderly) model. This role focuses on managing complex medical needs for an aging population through comprehensive assessments, care planning, and close collaboration with an interdisciplinary team. The ideal candidate is skilled in primary care, values continuity of care, and thrives in a collaborative environment. Responsibilities include conducting thorough evaluations, managing chronic and acute conditions, coordinating care across settings, and supporting safe transitions. This provider will actively participate in daily interdisciplinary team meetings, contribute to care planning, and ensure timely follow-up on labs, imaging, and specialist consultations. This role emphasizes thoughtful clinical decision-making, appropriate utilization of healthcare resources, medication management, and patient and caregiver education to improve outcomes and quality of life.
