Hospice/Palliative Medicine Nurse Practitioner

Home Health/Internal Medicine Nurse Practitioner (FNP or ANP)

Location: Greensboro & Burlington, NC (covering 5 surrounding counties)

Start Date: ASAP

Contract Length: 12 weeks (with potential to extend)

Schedule: Monday Friday, 8:00 AM 5:00 PM (40 hours/week)

Setting: Combination of Home Health and Telehealth

Position Overview

Seeking a Full-Time NP for a unique blend of home health and telehealth care, focusing on a geriatric patient population. This role includes conducting in-home visits and virtual appointments, providing primary and chronic care management for homebound patients. Ideal for a provider experienced in comprehensive geriatric care, chronic disease management, and patient education.

Key Responsibilities

• 6 home visits per day (appointments approx. 45 minutes)


• Up to 8 telehealth patients/day (equipment provided)


• Conduct physical exams, diagnose/manage chronic illnesses, perform advanced care planning


• Coordinate care with other healthcare providers


• Travel across a defined regional area within 5 counties (scheduling team sets daily route)


• Mix of new and established patients

Requirements

• Board Certified : Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)


• NC License and DEA : Required


• BLS : Must be AHA or Red Cross certified


• Experience :




• 2 3 years in acute care, long-term care, outpatient, or home health required


• Palliative care experience preferred




• EMR : Athena experience preferred (will need to complete training modules if unfamiliar)


• Travel :




• Must have valid driver's license and personal transportation


• Automobile liability insurance required
