Work Dates Needed: July 1, 2026 thru June 30, 2027, with the potential to extend.
Preferred Schedule: Monday - Friday; 8am to 4:30 pm.
Worksite Setting: Outpatient Clinic in GA
Scope of Work: Provider shall provide preventive care if immunizations, cancer screening, and lifestyle counseling. Chronic disease management of Diabetes, hypertension, COPD, heart failure, etc. Acute care evaluations and treatment of new or urgent symptoms. Mental Health screening for depression, PTSD, and substance use (with referrals as needed). Serve as the team leader in the PACT. Manage referrals, consults and follow up care. Educate veterans on nutrition, exercise, smoking, etc. Address aging-related issues. Women's Health: address reproductive health, menopause, etc.
Licenses, Certifications, Requirements: BE or BC in Internal Medicine, Any Active state license, DEA, BLS and ACLS
