SUBMISSION REQUIREMENTS Must have active NY license
Must have clean file and claims history
Must have ACLS, PALS, ATLS, ATLS- Must be through AHA- Hands on only
CME: 8 stroke
Health Requirements: All immunizations, TB, Flu, Covid
NY State License and DEA
