Guide interdisciplinary care teams and mentor providers
Oversee Quality Assurance/Performance Improvement (QAPI) efforts
Conduct assessments, treatment planning, and ongoing care management
Participate in intake evaluations and family conferences
Ensure compassionate, dignified care for aging adults in various residential settings
What You ll Need:
Active FL license
BC in IM/FM
DEA & CDS certification
1+ year experience working with frail or elderly populations
Prior leadership or supervisory experience in a clinical setting
