• Availability: Must be available for 15-20+ shifts per month

  The provider will be covering the Norther Colorado Region Hospitals: Medical Center of the Rockies, Poudre Valley Hospital, & Greeley Hospital

• Average number of patients per physician per day: 4-12


• Average number of hospital-based surgeries the locum tenens physician should expect to work: 3-6/day between 3 hospital locations


• Average number of office/clinic surgeries the locum tenens physician should expect to work: No clinic. Hospital only


• Percentage of the practice: Adults: 100%; Inpatient, 100%How many urologists in your Number of Urologist in the group: 10


• Mid-levels: 5 support primarily clinic/scheduled OR


• Lab & X-Ray: Yes


• No. of exam rooms: N/A - hospital call only


• Describe office equipment: C-arm, laser, robot


• Documentation: EPIC (hospital)


• How many hospitals will the locum tenens cover? 3 Name: PVH, MCR, GH (Greeley hospital)


• Describe referral/transfer protocol: No IR in Greeley on the weekends


• Describe office ancillary staff (e.g., number of RN's, LPN's, MA's, etc.): N/A - hospital call only; minor rounding support by APPS in the AM, please noon at Greeley/MCR
