Description: Mandatory Requirements:
- Board Certified (No BE), will accept IM/FM with minimum of 2+ years of recent ED experience - Required
- MI License - Required at time of clear, no exceptions - not accepting IMLC - Required
- Certification Requirements: ACLS, BLS, PALS - Required
- Provider must be local - REQUIRED, no exceptions
- Must have clean background for quick credentialing - REQUIRED no exceptions

Employee Health Requirement:
- a. Flu vaccine for 24-25 season
- b. Covid documentation - If 24-25 booster has not been received a declination is needed
- c. TB - 2 step skin test within 12 months or Quantiferon within the last 6 months
- d. TB Questionnaire
- e. Hep B - titer with quantifiable numbers or declination
- f. MMR - vaccine series or titers with quantifiable numbers
- g. Varicella - vaccine series or titers with quantifiable numbers
- h. TDAP - adult vaccine (within the last 10 years)
