Setting: Outpatient clinic
Responsibilities: New patient evaluations, follow-up visits, and minor therapeutic treatments (e.g., Botox, Occipital Nerve Blocks)
Required Procedures: Minimal Botox, ONB
EMR System: Epic
Support Staff: Dedicated LPN or MA
Days Per Month: Approximately 5 10 days/month depending on schedule gaps
On-Call/Weekends: None
Submission Requirements:
Board Certified or Board Eligible REQUIRED
Active WA license or IMLC REQUIRED
BLS and DEA REQUIRED
Clean malpractice history Preferred
60-day availability from projected start date REQUIRED
