Registered Nurse Case Manager Opportunity:
- Collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
- Coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
- The Registered Nurse Case Manager will monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care
Registered Nurse Case Manager Qualifications:
- Must be a licensed Registered Nurse (RN) or Licensed Social Worker (LSW/LCSW)
- Must have previous experience as a Registered Nurse Case Manager or Case Manager
