Board Certified/Board Eligible REQUIRED

Active CA license REQUIRED

GI Fellowship REQUIRED

Clean Malpractice strongly preferred

Availability at time of name clear REQUIRED

ACLS and DEA REQUIRED during credentialing

ERCP REQUIRED
