Qualifications:
- Must have outpatient primary care experience
- Position to start ASAP for a 6 month duration with possible extension
Job Duties/Details:
- Monday-Friday 8a-5p
- High elderly population
- Chronic Disease Management
- 36 patient care hours, 4 hours of admin built into schedule
- Average 16-20 patients per day
- RN and MA support
