You ll be providing exceptional care to patients in a post-acute care setting, utilizing your expertise in family medicine or internal medicine. Don t worry if you re fresh out of school we re excited to welcome new grads who are ready to learn, grow, and make a difference.

What You ll Do:

• Deliver top-notch care to post-acute patients Monday through Friday (weekends are for you!).


• Manage patient assessments, care plans, and follow-ups.


• Collaborate with a supportive interdisciplinary team that believes in quality care (and quality coffee).


• Document care with precision because every patient deserves a note-worthy experience.

• Certification as a Nurse Practitioner (CNP, FNP, APRN) or Physician Assistant (PA).
