September: 5, 12, 15, 16, 19, 22, 23, 26
September: 29-October 3 (entire week)
October: 10, 13, 14, 17, 20, 21, 24, 31
November: 7, 10, 11, 14, 21, 24, 25
December: 1, 2, 5, 8, 9,12,19, 26
Details/Requirements (Assignment # 108527):
Must have Indiana license and DEA
Board Certified in Hematology/Oncology
Monday through Friday, 8am-4:30pm
Outpatient setting, no call or rounding
Responsibilities: knowledge of NCCN guidelines, emergency response skills from side effects of medication, chemotherapy treatment plans
Patient volume: 20-25 patients per day
Support staff: Five RN's, 2 MA's, 1 Patient Service Representative
