Description: Adelphi is seeking dedicated and experienced LVNs/LPNs to provide full-time, on-site nursing care at a residential healthcare facility in Tom Green County, Texas. The ideal candidate will assist with medication administration, conduct patient assessments, support immunization initiatives, and deliver comprehensive health services for individuals with complex medical needs.

  • $200 sign on Bonus (Paid after 150 hours worked) Requirements

  • Completion of an Approved LVN/LPN Program

  • Active LVN or LPN license in the state of Texas

  • Minimum 2 years of experience in direct care preferred

  • Experience with dual diagnoses or behavioral challenges is a plus

  • Background in acute care, long-term care, community clinics, or public health preferred

  • Valid government-issued photo ID

  • Current CPR certification (American Heart Association only)

  • Negative PPD test (within past 12 months)

  • COVID-19 vaccination card, if applicable

  • Must pass a urine drug screen and agency-conducted background check

  • Must pass FBI fingerprint background check (conducted by the facility)

  • No extended time off allowed during holiday periods (Christmas and New Year's)

  • All compliance requirements must be met prior to start no exceptions Key Responsibilities

  • Administer medications in accordance with Texas policy and procedure.

  • Conduct initial and follow-up assessments for injuries and acute conditions.

  • Assist with TB skin testing, COVID-19 testing/boosters, and seasonal vaccinations (e.g., flu).

  • Document assessments and support implementation of healthcare plans.

  • Provide follow-up care and collaborate with case management teams.

  • Perform additional duties as assigned by nursing leadership.
