Are you passionate about connecting individuals experiencing mental health challenges with the services they need-before crisis escalates? Join our dynamic Front Door Diversion Team as an Outreach and Engagement Specialist, where you'll play a critical role in diverting individuals from emergency departments, inpatient psychiatric units, or the criminal justice system by providing early intervention, linkage to care, and compassionate support.







In this role, you will:


Provide field-based outreach to individuals in crisis or at risk of system involvement


Build trusting relationships and conduct needs assessments


Collaborate with behavioral health providers, hospitals, and property managers


Help clients access housing, medical care, behavioral health services, and other community resources


Support recovery-focused, person-centered care







What We're Looking For:


We're seeking candidates who are empathetic, culturally competent, and committed to equity in mental health care. Strong interpersonal skills and the ability to work independently in the field are essential.




Ideal Candidates May Also Have:


Experience working with individuals with serious mental illness, co-occurring disorders, or experiencing homelessness


Knowledge of local community resources and behavioral health systems

Requirements:

EDUCATION, SKILLS, and/or EXPERIENCE:



Bachelor's degree in Social Work, Psychology, Human Services, or a related field required


Master's degree (e.g., MSW, MFT, MHC) preferred for higher-level engagement or clinical support roles


Experience with outreach, case management, or crisis intervention is highly desirable




CADC License, IM/CANS certification, and knowledge of Medicaid standards; experience with crisis programs such as Acute Community Team (ACT), Community Support Team (CST) or First Responders programs; Knowledge of Northside providers and social services strongly preferred.

Compensation & Benefits:




  • Base Salary: $50,000 per year + additional quarterly productivity bonuses


  • Paid Time Off: 11 Vacation days, 12 paid Holidays, 3 sick days, 3 personal days


  • Illinois Paid Leave: 40 hours (accrued)
