Description: LPN - IDD - Pennsylvania - ICF Join Our Dedicated Team as an LPN in an IDD Program in Pennsylvania Are you a compassionate Licensed Practical Nurse (LPN) seeking a rewarding opportunity in an Intermediate Care Facility (ICF)? Be part of a dynamic environment dedicated to improving the lives of individuals with Intellectual and Developmental Disabilities (IDD). This role offers a chance to make a meaningful difference while advancing your nursing career within a supportive and professional setting. Key Responsibilities:
- Provide comprehensive nursing care to residents with IDD in an ICF setting
- Monitor health status, administer medications, and implement treatment plans
- Collaborate with a multidisciplinary team to deliver person-centered care
- Track and document patient progress accurately and timely
- Support residents' daily living needs, promoting safety and well-being
Required Skills:
- Valid Pennsylvania LPN license or license eligible
- Proven experience in Long-Term Care (LTC) settings
- Strong clinical assessment and problem-solving abilities
- Excellent communication skills, both verbal and written
- Ability to work independently and as part of a team
- Familiarity with state and federal healthcare regulations
Nice to Have Skills:
- Experience working with individuals with intellectual and developmental disabilities
- Knowledge of ICF regulations and protocols
- Bilingual abilities are a plus
- Certification in CPR and First Aid
Preferred Education and Experience:
- Licensed Practical Nurse with a current Pennsylvania license
- Minimum of 1-year LTC experience, preferably in an ICF setting
- Prior experience working with individuals with special needs
Other Requirements:
- Reliable transportation for travel within the facility or community as needed
- Willingness to work flexible shifts including possible weekends
- Certification in CPR and First Aid preferred
- Must comply with all health and safety regulations and policies
